Atrius Health, the Northeast’s largest nonprofit independent multi-specialty medical group, recently tested the benefits of care coordination in an affiliated skilled nursing facility by implementing embedded care coordinators.
While outcomes were already positive, further improving efficiency and reducing readmission rates was still a primary goal. The care coordinators at Atrius adhered to defined protocols to ensure patients followed specified care plans and post-discharge procedures.
Overall, the results were phenomenal. Steven Strongwater, MD, President and CEO of Atrius Health, noted a significant decrease in both length of stay and cost savings:
So, we cut that metric [length of stay] almost in half and the readmission rate, which was in the 25% range, was down to 8%. And for us, a two-day length of stay reduction is worth about $2 million … it was a great investment for us.
